To obtain spa services, all NEW patients can elect to come and see Dagmar for an initial 30 minute complimentary skin care consultation  to determine what treatments are best for your skin type and to make sure there are no medical contraindications to the treatment you are seeking.

For returning patients - You will be able to make your own appointments in the patient portal or call our number to have the receptionist assist you.

You can make an appointment to come to the spa one of two ways:

A. You can call our number, 703-272-8501, from 8 am to 8 pm M-F and our receptionist will walk you through the process or make appointment for you. OR

B. Go to the following hot link to fill out your demographics and make an appointment.

1. Follow the prompts on https://lwc.myemedfusion.com/epp/ to fill out your demographic information and create a user name and password.
2. Next, click on the box that asks if you only want an aesthetic appointment and click that button. (Patients for the wellness program, weight loss clinic and hormone therapy would not click the aesthic button and would go on to fill out their history & HIPAA forms and make the initial payment for lab work to get done and evaluated by the doctor).
3. Once you have created your demographics page - go to the website athttps://longevitywellness.net/spa_forms.php and click on the appropriate history form and print it out. Complete the form and bring it with you on your first visit. The Spa Consent and History is for all spa procedures. The additional form for massage should also be filled out for all who are obtaining massage therapy as it has specific questions that deal with those procedures.
4. You can then make an appointment through the portal https://lwc.myemedfusion.com/epp/ or you can call our number, 703-272-8501 and have the receptionist make your appointments. If you are a new patient, you must choose the Aesthetic consult for 30 minutes with Dr. Petitt which is complementary. If you wanted to get your procedure done that day, you can also make an appointment after your consultation with Dr. Petitt.
